Moisture in Foundation

We have a couple of locations in our home where the foundation pegs out on a moisture meter.  We first noticed an issue when our hardwood floors started blistering.  At first we thought we had spilled something on them, so we replaced those specific boards, never seeing any signs of moisture.  We have seen another area of flooring that is dis-coloring.  The house is 20 years old and we have now had several leak detection companies and plumbers and can find no reason for the moisture.  We are getting ready to do the renovations but do not want to move forward until we get this issue addressed.  We want to find the reason for the moisture issue and we also want to continue to have wood floors.  Any ideas or suggestions?
